    Description

    Position Profile: Specialist: Care Manager

    Position:

    Specialist: Care Manager

    Grade:

    B4M1

    Position Purpose:

    To educate and support members with the aim of empowering them to manage their disease better thereby improving their health outcomes and limiting healthcare costs whilst maintaining quality of care.

    Experience:

    2 - 3 years nursing in a hospital or clinic experience, providing patient care

    Qualifications:

    Nursing diploma or degree; fully qualified and registered Nurse

    Position Specific Outputs:

    Proficiency in the following languages: Sotho, Venda and Tsonga
