Job Summary:

The Data and Linkage assistant will be responsible for routine data management and linkage functions at a facility level. S/he will also be the point of contact for data collection, collation, verification, analysis, and generation of reports. They will also be responsible for consistent feedback to the clinical teams on identified gaps and poorly performing indicators. The purpose for this position is to carry out routine data management functions and ensuring that routine linkage and retention operations/activities are implemented as per the standard operating document or guidelines

Will be placed at either clinics at uMgungundlovu:

  • Caluza Clinic
  • East Boom CHC
  • Mason Clinic

Essential Duties and Responsibilities include the following:

1. Data assistant role

  • Capturing and updating all ART patient's visits on and share the reports with the clinic staff
  • Ensure all blood tests and results are timely updated on
  • Work with the clinic team to ensure prompt identification and actioning of line lists such as missed appointments, lost to follow up clients, unsuppressed viral load and viral load overdue lists and any other required line lists.
  • Collection, collation and verification of HIV preventions and care & treatment data from AHF supported sites.
  • Preparation and timeous submission of weekly, monthly and quarterly reports to AHF and DOH as per reporting guidelines and schedule
  • Prepare and analyze data required for monthly and quarterly feedback meetings.
  • Ensure that performance charts and indicator barometers are updated and displayed to monitor program performance.
  • Ensure the availability of data collection tools and proper documentation in all tools.
  • Conduct data quality checks including validation, verification and data clean up to ensure the quality of data
  • Support all site level quality improvement activities to improve program performance
  • Ensure the availability of data collection tools and proper documentation in all data collection tools.

2. Linkage assistant role

  • Document linkage data and ensure at least 90% linkage to care of all HIV positive clients as per AHF's linkage definition.
  • Support the clinic team to trace, contact and bring to care those clients that are urgently required to return to the clinic e.g. those with unsuppressed viral load, positive TB test etc.
  • Conduct telephone tracing, arrange home visits and work with community structures, including WBOTS to reach newly diagnosed HIV positive clients who haven't been linked to care and treatment, and document in the appropriate linkage and tracing tools
  • Work with the clinic team to trace and bring to care those patients who missed their appointments or are in the lost to follow up lists.
  • Work with the clinic team to follow up on contacts of HIV Index patients who have not presented to the clinic following contact elicitation and notification.
  • Perform any other duties assigned.

SUPERVISORY RESPONSIBILITIES

  • None

EDUCATION AND EXPERIENCE

  • Diploma or Bachelor's degree in Health Sciences, Social Sciences or Health Information Sciences.
  • A minimum of three years' experience working within the HAST program,
  • Knowledge of systems used in the South African public health setting including ETR, and DHIS.
  • Good knowledge of all HAST programme cascades utilized in the capturing and monitoring of data.
  • Computer literacy (Ms Office)

KNOWLEDGE, SKILLS AND COMPETENCIES:


  • Basic knowledge of HIV/AIDS, TB and STI's programs
  • Data capturing skills
  • Good numeracy and communication skills
  • Strong interpersonal and communication skills

LANGUAGE SKILLS

  • Fluency in English and any other South African language;
  • Ability to read, analyze, and interpret documents such as safety rules, operating and maintenance instructions;
  • Ability to effectively present information to team members in a clear and concise manner
  • Ability to write routine reports and correspondence;

Mathematical Skills:


  • Ability to add, subtract, divide and multiply in all units of measure, using whole numbers, common fractions, and decimals;
  • Ability to compute rate, ratio, and percentage, and to draw and interpret bar graphs, line charts and pie charts.

Computer skills:


  • Excellent computer skills;
  • Proficiency in MS Word, MS Excel, MS PowerPoint and internet;
  • Maintains 95% accuracy with data entry.
