Caring for an 67 year old male patient with mild stroke, speech problem, high blood pressure prostate cancer and also crippled, giving care support that including bathing, dressing, feeding, and bed making.
Assisting patient in every step of testing and screening and providing them with relevant education, which increases positive feedback.
Administering insulin and checking sugar levels.
Pressure area and wound care.
Damp dusting the sick room to ensure clean and hygienic environment put infection under control.
Exercising recommended lifting, moving, and handling techniques to the patient
Applying  first aid treatment to residents where necessary, for example, on miscellaneous accidents like, burns, nose bleeding and insect bites.
Changing diapers/pads per rising need taking note of skin integrity. Applying moisturizers and barrier creams to maintain good skin integrity.                                                                                                                                   
Undertaking care planning activities for the patient to meet the changing physical, psychological and social needs taking into consideration, privacy, dignity, personal choice, and informed consent.
Spearheading risk assessment of patient and his relatives, developing individualised care plans, implementing, and evaluating them this enhance working relations.
Reminding and assisting the patient on dates for Doctor’s review and it increases adherence.
Assessing and planning for the educational needs of the patient.
