The coordinator plays a key role in an integrated funder-provider program designed to co-ordinate the care of a carefully selected group of DH members by actively identifying patients that meet our clinical entry criteria.

The Coordinator facilitates the care of these patients in an appropriate facility in collaboration with the referring doctor, patient, family and the Scheme.

This requires the building and maintaining of excellent relationship with facility staff and patients.

The aim of Care Coordinator's role is to ensure that sustainable long term care plans are in place by the end of the rehab admission.

This will be done in conjunction with the family and the facility.- Aim- Actively enabled a thorough discharge plan including, handover to relevant providers in the OH setting.

Specific action plan outlined to the family for problems that may arise post discharge. Social interventions in place as required. Ongoing care and management put in place for the member.- Key output:

  • The successful applicant will be responsible for but not limited to the following job functions:

  • Perform health assessments on patients in order to identify individual patient risk factors
  • Work closely with the facility promoting the uptake of CCP
  • Maintaining relationships between the acute hospital, the specialists and the rehabilitation facility and Discovery Health

Other responsibilities include:

  • Disease education to promote treatment compliance and condition management
  • Attendance of multidisciplinary team meetings and family meetings
  • Assist with benefit navigation and access
  • Assisting in obtaining discretionary funding for members outside of plan benefits
  • Facilitate a handover to the Member Care Program for further out of hospital management

  • Qualification & experience:

The following requirements are essential:

  • Matric
  • Registered Nurse or Enrolled Nurse(working under supervision of RN, colleague in the same region)
  • 2 years Discovery Health experience
  • MCP or AIB
  • Valid SANC Registration
  • 5 years industry experience with a minimum of 2 or more of the following:
Clinical Nursing Experience
Care Management or similar experience
Multidisciplinary team experience

  • Valid driver's license and own transport
  • Microsoft Office
