Key Purpose:
To holistically review the needs of mental health users and coordinate their care in the most appropriate setting, liaising with internal and external stakeholders.

- Located in international guidelines, the framework of the _Mental Health Care Act No. 17 of 2002 _and other core South African legislation and mental health policies
Key Outputs:
- Coordinate the care, treatment and rehabilitation of identified mental health users across the health care journey to improve health outcomes
- Conduct screening for depression and enroll members on the Mental Health Relapse Programme
- Promote mental health in all settings and to all people, addressing issues of stigma and mental wellbeing.
- Interacting with GP's, Psychologists and Psychiatrists in order to develop and implement member care plans as a central part of the Mental Health nurse role and as a part of the counsellor role as well.
- Demonstrate an indepth knowledge of theories, which underpin mental health practice.
- Collaborate with the mental health care user, his/her family, the community and the multidisciplinary team to conduct evidencebased, comprehensive mental health assessments; to diagnose, prioritise, stabilise or refer as appropriate; and to develop collaborative diagnosis care and treatment plans.
- Demonstrate evidence of sound clinical judgement and clinical reasoning, reflectioninaction, drawing on indepth knowledge of the aetiology of mental illness and psychopharmacology, to initiate appropriate management.
- Establish therapeutic relationships with mental health care users, their families and the community.
- Conduct evidencebased mental health, and social or psychotherapeutic interventions, inclusive of the use of mHealth (mobile) / eHealth (digital).
- To document all member interactions as well as to ensure that all records are accurate and uptodate
- Exercise an advocacy role in care, treatment and rehabilitation, with a recovery
- Monitor patient progress through ongoing patient interaction and followup calls
- Manage referrals for counselling
- Load baskets of care
- Medication and chronic disease review

Qualifications and Experience:
The following requirements are Essential:
- Matric
- Registered Nurse qualification with mental health specialization
- Valid SANC registration
- MS Office
- At least 3 years clinical nursing experience, post training.
- 2 years mental health nursing experience
