    As a Patient Care Coordinator, you will play a vital role in ensuring seamless transitions for our patients and families. Your primary responsibility will be to assess patient needs and develop comprehensive discharge plans in collaboration with our multidisciplinary team.

    We are seeking an individual with excellent communication skills and a passion for delivering exceptional patient care. The ideal candidate will possess a strong understanding of community resources and social services.

    We offer a competitive salary range of $60,000 - $80,000 per year, depending on experience.

    The successful candidate will have a Master's degree in Social Work (LMSW), Clinical Social Work (LCSW), or Mental Health Counseling (LMHC) and hold current certification in New York State PRI assessment.

    Diverse candidates who can speak Spanish or other community dominant languages are highly valued.
