About the Role

The successful candidate will provide one-to-one care to patients in their homes or in a facility setting.

Key Responsibilities

• Provide basic care, including assisting with personal hygiene (washing, dressing, grooming, feeding, and toileting)
• Administer medications under the supervision of a registered nurse or family member
• Document patient condition and progress
• Assist with activities of daily living (ADLs) for patients
• Monitor patient vitals and record
• Perform basic wound care
• Manage feeds for tracheostomy patients using a feeding pump

Requirements

• A strong understanding of treatment plans set out by other team members (Doctor, NSM, or Physiotherapist)
• Good communication skills with supervisors, colleagues, patients, and family members
• Knowledge of emergency situations with patients in their homes
• Geriatric Care, Paediatric Care, Dementia Care, Post Stroke Care, Ventilation Care
• Catheter care (indwelling, supra pubic, intermittent)
• Prevention of pressure sores in bedridden patients
• Trachea care or experience working in a unit with tracheostomy patients
• Suctioning of a trachea patient
• Prevention of infection in all patients
