Job Advert Summary:


Purpose of the position:


A Case Manager provides oversight and supervisory role to Case Officers toward the achievement of specific program key performance areas, with a focus on retention.

This cadre will provide support to achieve optimum adherence and retention for HIV-diagnosed patients until they are eligible to be decanted.

Additionally, this cadre will offer general administration support, data capturing, collation, and reporting as required.


Minimum Requirements:


  • Matric
  • Postmatric qualification (M&E, Basic HIV training, or experience in lieu of training)
  • A minimum of 35 years' relevant experience in the health sector, managing or implementing HIV/AIDS and STI (HAST) programmes with knowledge of HIV and TB treatment cascades
  • At least 2 years' experience in supervisory/managerial and mentoring or coaching role at facility level
  • Relevant public health experience (hospital and primary health care level), the NGO environment will be advantageous
  • Advanced computer literacy skill (e.g. MS Office package, SyNCH, Tier.
Net, SharePoint, etc.)

  • Basic knowledge or training in project management
  • Valid driver's licence
  • Clear criminal record
  • Able to communicate in English and isiXhosa

Duties and Responsibilities:


  • Developing relevant tools and enforce use of recognised tools and registers
  • Support documentation of activities on recognised source documents
  • Identify and name key participants and their roles in the facility Case Management Model
  • Supervision and mentoring
  • Support Case Officers to document appointment dates on spreadsheets and allocate dates of return for all care milestones
  • Coordination, monitoring & evaluation
  • Establish processes/systems
  • Outline the monitoring system, targets and standards for each Case Officer
  • Report writing
  • Stakeholder engagement at facility level
