**Job Summary**

The successful candidate will work one-on-one with patients in the comfort of their own homes or provide care for a patient in a facility, offering personalized support and assistance.

Minimum Requirements:

• Work as part of a multidisciplinary team, caring for adults and children with tracheostomies connected to ventilators, elderly patients with a focus on dementia and Alzheimer's care, quadriplegic patients with PEG tubes, and palliative patients nearing the end of life.
• Provide basic care, assist with personal hygiene tasks such as washing, dressing, grooming, feeding, and mobility support, as well as toileting.
• Administer medications under the supervision of a registered nurse or family member.
• Hold a First Aid Certificate, which is advantageous.
• Document patients' conditions and progress.
• Support and assist patients with activities of daily living (ADLS).
• Monitor patients' vitals and record them.
• Perform basic wound care.
• Manage feeds for a tracheostomy baby using a feeding pump.
• Observe changes in patients' conditions and record them in the daily report.
• Write reports in good English.
• Assess patients' sleep routines, record, and discuss with registered nurses to adjust care plans accordingly.

Skills and Knowledge:

• Understand treatment plans set by other team members (Doctor, NSM, or Physiotherapist) to ensure all tasks are completed appropriately.
• Develop strong communication skills with supervisors, colleagues, patients, and family members.
• Deal with emergency situations involving patients at home.
• Maintain knowledge of geriatric care, pediatric care, dementia care, post-stroke care, ventilation care, catheter care, and pressure sore prevention in bedridden patients.
• Have good knowledge of trachea care, having worked in a unit with tracheostomy patients or observed.
• Suction trachea patients.
• Prevent infections in all patients.
• Skilled in writing reports.

Desired Qualifications:

Diploma as a qualified Enrolled Nurse.
