Job Advert Summary:


Purpose of the position:

Responsible for comprehensive, integrated cascade of care, inclusive of diagnosing, treating, monitoring and evaluating communicable and infectious diseases.

The PN will consult and provide preventative, curative and rehabilitative treatment/health care to people on various clinical aspects (Adult Curative care inclusive of communicable, non-communicable diseases).

Supports Maternal and Child Health (MCH) as determined by DoH HR presence to ensure optimal service delivery. They will work collaboratively with outreach teams and contracted community-based organisations (CBOs) in monitoring adherence of decanted patients, e.g. reporting non-collection of medicine parcels at Central Chronic Medicine Dispensing and Distribution (CCMDD) pick-up points. Support any other activities related to COVID-19 vaccination and awareness.


Minimum Requirements:


  • Bachelor's Degree or Diploma in Nursing Sciences in health sciences
  • A qualification in Primary Health care (PHC) or clinical assessment will be an added advantage
  • Current registration with the South African Nursing Council (SANC) as a Professional Nurse
  • NIMART trained and certified
  • Adult Primary Care (APC) is essential
  • Dispensing licence will be an added advantage
  • A diploma in HIV Clinical Management and /or NIMART
  • At least 2 years' experience in nursing post registration
  • A minimum of 2 years' experience in HIV and TB programs in the health sector implementing HIV/AIDS and STI (HAST) programs with knowledge of HIV and TB treatment cascade
  • Experience with NIMART, CCMDD, Sexual Reproductive Health (SRH), Maternal and Child Health (MCH)
  • Previous exposure in NGO environment will be an added advantage
  • Additional training in HIV/AIDS, TB and STIs, communicable diseases and NCD management would be an added advantage
  • Able to communicate in English and isiXhosa will be an advantage
  • Valid driver's license
  • Clear criminal record

Duties and Responsibilities:


  • Provide services using evidencebased HIV and TB interventions in all streams according to the National and Provincial Department of Health (DoH) policy, guidelines and standard operational procedures (SOP)
  • Optimise HIV Testing Services (HTS) at facilities using different modalities such as Index Testing Services (ITS) and HIV Self-Screening (HIVSS) in identified target areas, and link them into care including TB diagnosed patients as per NDoH ART and TB Management guidelines
  • Implement the Universal Test and Treat (UTT) policy according to the latest guidelines to support attainment of UNAIDS targets
  • Monitoring referrals and ensuring successful referrals are validated
  • Support the Ideal Clinic Realisation and Maintenance (ICRM) programme and quality improvement interventions at the facility
  • Implement an effective appointment system and Differentiated Care models of the decanting programme
  • Promote patient decanting of stable patients by creating demand for the CCMDD service and TLD transition and identifying eligible patients
  • Participate in facility QIP using bottleneck analysis
  • Keeping abreast with new clinical developments e.g. training, inservice training, reading journals, policies, and procedures related to job
  • Implement case management and support patient retention in HIV and TB programme
  • Participate in COVID19 screening, referral and ensure compliance with infection control measures
  • Record and trace patients interrupting treatment working with Coaches and Case Officers and verify lists of not confirmed as linked to care
  • Conduct weekly reviews to clear ART waiting patient lists and trace clients (adult and paediatric) for treatment initiation
  • Review cohort reports and facilitate tracing of PCRpositive babies and HIV positive women for treatment initiation
  • Monitor viral load uptake and suppression using eLabs, works with Coaches and Case to ensure bloods are taken at due intervals, decant once VL suppression is achieved, refer for enhanced adherence counselling (EAC) or for secondline treatment regimens consideration
  • Develop an effective system to monitor viral load tests done on a weekly and monthly basis and results for action (rFA)
  • Scale up CCMDD patient registration and rescription on the electronic SyNCH system
  • Identify and document good facility best practices from implemented Quality Improvement Plan (QIP) activities on a continuous basis and work with QI team to package them
  • Participate in data analytics (actual performance on achievement of facility targets, tracer indicators, cascades)
  • Submit daily, weekly and monthly data internally and to the facility and as required
  • Participate in the facility's biweekly and monthly data feedback and review meetings
  • Ensure staff understands the contents of standard operation procedures and implements accordingly
