    Summary

    This is a challenging and rewarding role that requires a strong understanding of patient care, particularly in complex cases. You will work closely with patients and their families to provide high-quality care and support.

    Responsibilities

    Direct Patient Care:

    • Provide basic care, including personal hygiene assistance (washing, dressing, grooming, feeding, toileting)
    • Administer medications under the supervision of a registered nurse or family member
    • Document patient condition and progress
    • Assist with activities of daily living (ADLs) for patients
    • Monitor vitals and record
    • Perform basic wound care

    Care Planning:

    • Assess sleep routines and record for discussion with registered nurse to adjust care plan
    • Observe changes in patient condition and record in daily report

    Treatment Plans:

    • Understand treatment plans set out by other team members (doctor, NSM, physiotherapist) to ensure tasks are completed appropriately

    Skills and Knowledge:

    • Good communication skills with supervisors, colleagues, patients, and family members
    • Knowledge of emergency situations with patients at home
    • Geriatric care, paediatric care, dementia care, post-stroke care, ventilation care
    • Catheter care (indwelling, supra pubic, intermittent)
    • Prevention of pressure sores in bedridden patients
    • Trachea care or experience working with tracheostomy and peg patients
    • Suctioning of trachea patients
    • Infection prevention in all patients

    Qualifications:

    • Diploma as a qualified enrolled nurse
