    Registered Nurse Specialist: Disease and Programme Management

    Our client, a leading medical recruitment agency in South Africa, has an exciting opportunity for a Registered Nurse Specialist: Disease and Programme Management based in Pretoria.

    This role involves delivering active disease management, care support, coaching, and health education services to medical scheme beneficiaries with chronic conditions.

    The ideal candidate will be part of the clinical and disease management teams, coaching beneficiaries on achieving and sustaining health outcome goals while reducing healthcare costs.

    Main Responsibilities

    • Integrate pathophysiology, epidemiology, clinical management, and self-management of chronic conditions into clinical practice.
    • Advocate for improved quality of care and outcomes for those living with or affected by chronic conditions.
    • Conduct comprehensive assessments, including emotional and behavioral health evaluations.
    • Develop individualized care plans based on patient needs and goals, promoting successful self-management.
    • Facilitate follow-up and review of pathology tests and interventions with patients and treating providers.
    • Document all individual contacts and outcomes data in accordance with guidelines.
    • Identify partially controlled patients and intervene appropriately.
    • Participate in meetings and assist team members with hospital admissions and patient outreach efforts.
    • Prepare reports and case studies as required.

    Requirements

    • Registered Nurse with postgraduate certification or diploma in counseling and/or psychology advantageous.
    • Valid SANC registration and up-to-date qualifications.
    • At least 3 years' experience in Disease Management, clinical patient management, and health education within a Managed Care environment.
    • Oncology case management experience advantageous.
    • Maternity wellness program experience advantageous.
    • Dispensing licenses for nurse practitioners with a focus on non-communicable diseases advantageous.
