The AIDS Foundation of South Africa (AFSA) is a Durban-based NGO that acts as an interface between Donors and Community Based Organizations (CBO's) working in the HIV/AIDS sector by placing donor funds with strategically selected CBOs, in South Africa, and providing them with ongoing, technical support & capacity building.

Purpose of job:

Support communitybased case management

Roles and responsibilities include but are not limited to:

Supportive supervision of community care workers
Support communitybased case finding and management
Monitor correct use, completion of case management and data collection tools
Assign daily and weekly caseloads to community care workers
Supervise, monitor and support field staff ensure proper data recording and reporting daily, weekly, monthly
Monitor OVC Preventive activities
Monitor quality of EBI implementation and completion of curriculum
Attend facility relevant meetings and community meetings to strengthen OVCY FSP implementation
Collect completed patient data from CYCW and submit to district for capturing and filling
Contribute to successful case identification and assist in writing/ reporting best practices
Support program data validation
